The incident note is created in your browser. This site has no account, analytics, server form, evidence upload or case database. The site does not intentionally save form contents, but your browser may temporarily restore them after reload or session recovery. Close the tab and clear site/browser data on a shared device. The language and city choices are the only user-specific values deliberately stored, on this device only.

What leaves this site

Calling a number, opening an email address, visiting an official portal, copying, downloading, printing or sharing moves information to your phone, browser, provider or the selected authority under their rules. Review every recipient and attachment.

Hosting and offline files

GitHub Pages and network providers may process standard request logs. A supported browser may separately cache the public application files for offline use. The application cache contains the website, not information entered into the form. Clear this site’s browser data to remove the public cache and language choice.

Use safely

Do not enter Aadhaar, PAN or full identity numbers. Do not publish live locations, medical records, detainee details or identifiable images without consent. Avoid shared or untrusted devices for sensitive matters.
